Transport Request Form

Before filling out this request, please read the Basic Requirements for Transports.

Update December 2, 2023: we are currently not processing anymore transport requests until after January 1, 2023. We will still be excepting them just not processing them. Thank you and have a safe, healthy and happy holiday season.

Transport wait times are currently 3 weeks from the time we receive your request.

If you have an urgent or emergency transport, please email us at imaginehomeorg@gmail.com and an admin will get back to you to see how we can help you.

*We ask that all cats being considered for transport, be at least 16 weeks of age and no older than 12 years old (without written acknowledgement from vet that the senior kitty is healthy enough for travel for the length of time that they will be on the road. )

*Please note we are currently not accepting transport requests that will take more then 30 hours to complete. If you have a need for a transport that will take more than 30 hours please email us at imaginehomeorg@gmail.com. Thank you so much for your understanding.

*Disclaimer: Imagine Home Inc. officers, Board of directors, administrators, and volunteers will not be held responsible for any illness, injury, damages incurred during transports.

**Since we request participation in transports from adopters and fosters, please know that your transport request implies your agreement with these Covid 19 social distancing protocols.

Important Notice

This transport will progress more smoothly when all parties are on board and willing to work with Imagine Home. Please submit the names of everyone on both ends of the transport who will be helping to coordinate things. If any changes need to be made, including, but not limited to cancellation of transport, you must agree to keep Imagine Home informed in a timely manner. Failure to do so within 24 hours may cause the transport to be cancelled.

Imagine Home requires an Adopter's Vet reference. Please submit contact information for Adopter's Vet along with name on account with vet. If Adopter currently has no other pets, please provide name & contact info of most recent Vet. *Please let your vet know we WILL be contacting them. Give them permission to speak with us. This will save us all time.
